Application Process
To make a complete application for this gift you will need to:
- Fill out the application questions below.
- Upload 1 form of ID (see guidance on acceptable ID)
- Upload medical proof of cancer diagnosis – This can be either:
a) a letter from your medical team confirming you are receiving chemotherapy/ radiotherapy/ surgery for the treatment of cancer, or have recently completed treatment (within the last year or so) or
b) have your nurse/doctor fill out and sign this downloadable medical form. You can then scan or take a photo of the completed letter/form and upload when you make your application. - Submit by clicking the tab below.
Once you submit everything, we will process your application and let you know within a few days whether you were successful. All documents are treated highly confidential and once you have been informed, we will delete your ID and medical documents from our files in line with our data protection policies.
To apply for a gift, you are required to:
1) Upload formal photo ID (not a selfie or photo)
2) Upload a Medical Condition Verification letter confirming your diagnosis
You will have the opportunity to upload these images as you fill out your gift application form.
Please see the details below for the specific Photo ID & Verification requirements. We are unable to process any applications that fail to provide the two required forms of identification.
What is ‘acceptable’ photo ID?
Please note that photo ID does not mean a ‘selfie’ or a ‘holiday snap’ and we certainly do not require a photo which highlights that you are receiving treatment (we get asked that question daily).
Photo ID can be a scan of your driving license or your passport.
